Title: A Randomized Clinical Trial of Oral Analgesics Utilization for Childhood Musculoskeletal Injuries (OUCH Trial)

Project Summary & ImpactMusculoskeletal injuries (MSK-I) in children are very common and almost universally painful. Pain management of MSK-I in the Emergency Department (ED) varies greatly between Canadian hospitals and, overall, pain is poorly treated. This sub-optimal pain treatment can have significant acute and chronic negative effects. Previous studies have determined that monotherapy with ibuprofen, one of the most commonly prescribed oral analgesics in the ED, is likely providing inadequate pain management for children. To date, few studies have focused on the efficacy of a combination of an anti-inflammatory drug (eg ibuprofen) and an oral opioid (eg morphine). Such a combination of analgesics is known to potentiate pain relief by blocking pain at the level of both the peripheral and central nervous system. To our knowledge, no previous studies have looked at the efficacy and safety of this combination of medications in the pediatric ED.

 

Primary aims and objectives:   To assess the efficacy of a combination of oral morphine and ibuprofen for acute pain management in children with MSK-I in the ED.

Primary outcomes Pain intensity score < 30 mm on the VAS at 60-minutes after treatment administration (T-60)

Secondary outcomes Oxygen saturation, level of sedation/alertness, and vital signs were monitored at set time points in the study.
